More Than Getting Out โ€” Building a Life in the Community

Social isolation is one of the most significant barriers to wellbeing for people living with disabilities, mental health conditions, learning difficulties, or the challenges of older age. Community access and social inclusion support is not a luxury โ€” it is a fundamental part of good care.

At Mother Care East London Ltd, our support workers help individuals to access the activities, services, relationships, and opportunities that matter most to them. We do not prescribe what that looks like โ€” we listen to the individual, understand their goals, and work creatively and consistently to help them achieve a fuller, more connected life.

All community access support is built into the individual’s care plan and delivered by a consistent, matched support worker who genuinely knows and invests in the person they are supporting.

Our community access support helps individuals to get out of the home and take an active part in their local community. From attending medical appointments and college to shopping, leisure activities, and exploring their neighbourhood โ€” we provide the support, confidence, and practical assistance needed to make it happen.

From First Call to Getting Out โ€” Four Simple Steps

Our process begins with listening โ€” understanding the individual’s goals, interests, and circumstances before we do anything else. Everything that follows is built around what matters most to them.

Get in Touch

Call us on 07510 916481 or complete our referral form. We discuss the individual’s current situation, what they would like to do, and what barriers are currently getting in the way of community participation.

Goals and Needs Assessment

We meet with the individual โ€” and their family or care team where relevant โ€” to understand their goals, interests, support needs, and any specific considerations around safety, communication, mobility, or health.

Support Worker Matching

We match the individual with a support worker based on shared interests, personality, communication style, language, cultural background, and the specific activities they want to pursue.

Support Begins and Goals Are Reviewed

Community access support begins to the agreed plan. We review goals regularly with the individual โ€” celebrating progress, adapting the support as confidence grows, and always working towards greater independence.

Community Access and Social Inclusion FAQs

How do you decide what activities to support someone with?

We start by listening. During the initial assessment, we take time to understand what the individual enjoys, what they have always wanted to try, what matters to them, and what barriers they face. The support plan is built entirely around their goals and interests โ€” not a standard list of activities. We review and update this regularly as the individual’s confidence and goals evolve.

Can the support worker help with travel as well as the activity itself?

Yes. Our support workers accompany individuals door-to-door โ€” from home to the activity and back. We support travel by foot, bus, tube, or taxi, and we work with individuals to build their own travel confidence over time through travel training and graduated independence where appropriate.

What if the individual wants to try something new that is not on their current plan?

That is exactly the kind of ambition we want to encourage. Our support plans are living documents and we regularly review and update the activities and goals included in them. If an individual wants to try something new, their support worker raises this with our coordination team and we work to incorporate it into the plan.

Can community access support be combined with other care services?

Yes. Many individuals we support receive personal care, medication management, or other services from us alongside their community access support. All services are co-ordinated through a single, joined-up care plan to ensure consistency, avoid gaps, and make the best use of the individual’s time and energy.

How do you keep the individual safe in the community?

Safety in the community is carefully assessed for every individual before support begins. We carry out a risk assessment that covers transport, specific activities, communication needs, medical considerations, and emergency protocols. Support workers carry emergency contact information and are trained in safeguarding, first aid, and risk management.

Is community access support funded by the local authority or NHS?

Community access and social inclusion support can be funded through local authority social care assessments, personal budgets, direct payments, NHS Continuing Healthcare, and personal health budgets. It may also be arranged and funded privately. We are happy to support families in understanding what funding they may be entitled to.
