About This Service
Support That Puts You in Control of Your Own Home and Life
Supported living is fundamentally different from residential care. The individual lives in their own home โ renting it themselves or through a housing provider โ and receives the support they need to manage daily life on their own terms. There is no institution, no shared timetable, and no loss of autonomy. The home belongs to the individual, and the support wraps around them.
At Mother Care East London Ltd, we co-produce every supported living package with the individual โ starting with what matters to them, what their goals are, and what kind of life they want to lead. Support workers are carefully matched, consistently assigned, and trained to promote independence rather than dependency.
We work closely with local authority commissioners, housing providers, care coordinators, and the individual’s family to ensure the whole arrangement is stable, well-funded, and genuinely built around the person at its centre.

Floating Support
Flexible support in your own home or tenancy
Floating support provides flexible, time-limited or ongoing support to individuals in their own homes or tenancies โ helping them to develop the skills and confidence needed to maintain their tenancy, manage daily life, and achieve their personal goals. Support is delivered at agreed times and can be increased or reduced as needs change.

Intensive Supported Living
Around-the-clock support for complex needs
Intensive supported living provides higher levels of support โ including waking nights and 24-hour cover โ for individuals with more complex needs who require consistent, skilled assistance throughout the day and night. Despite the intensity of support, the individual remains at the centre, retaining choice and control over every aspect of their home and daily life.

How We Start
From Referral to Moving In โ Four Steps
Setting up a supported living package takes careful planning. Our process is thorough, collaborative, and centred on the individual’s voice at every stage.

Referral and Initial Discussion
Contact us by phone on 07510 916481 or via our referral form. We discuss the individual’s current situation, support needs, goals, and housing arrangements โ and we liaise directly with the local authority commissioner or care coordinator from the outset.

Needs Assessment and Support Planning
We carry out a thorough assessment with the individual โ exploring their goals, daily living skills, support needs, communication preferences, and what a good life looks like for them. The support plan is co-produced with the individual at its centre.

Support Worker Matching and Introduction
We match the individual with a small, consistent team of support workers whose skills, personality, and approach are well suited to them. We introduce the team before the package begins so that trust is established before day one.

Support Begins and Goals Are Reviewed
Support commences to the agreed plan. We review outcomes regularly with the individual, their care coordinator, and family โ celebrating progress, adapting the support as independence grows, and always keeping the individual’s goals at the centre.

Supported Living FAQs
What is the difference between supported living and a care home?
In supported living, the individual lives in their own home โ they hold or contribute to their own tenancy and have full rights as a tenant. The support comes to them, rather than them going into a service. This means the individual has genuine choice and control over who supports them, how their day is structured, and how their home is run. A care home, by contrast, is a registered establishment where the provider controls the environment. Supported living is rights-based and person-led in a way that residential care cannot be.
How is a supported living package funded?
Supported living packages are typically funded through two separate streams: the housing costs are covered by housing benefit or universal credit (paid to the housing provider), while the support costs are funded through the local authority’s social care budget or, where eligible, NHS Continuing Healthcare. Some individuals use personal budgets or direct payments to commission their own support. We work directly with commissioners and care coordinators to establish and maintain the funding arrangement.
Who chooses the support workers?
The individual has a say in who supports them. During the setup process, we share profiles of potential support workers, arrange introductions, and take the individual’s preferences seriously. If a support worker is not a good fit, we work to find a better match. We also consult with the individual when any change to their team is proposed โ their comfort and trust in their support workers is our highest priority.
Can the level of support be reduced as someone becomes more independent?
Yes โ and this is something we actively plan for. From the outset, the support plan includes independence goals and a framework for reviewing and reducing support as the individual develops skills and confidence. We report on progress at every review and work with the commissioner to adjust the package accordingly. The goal is always for the individual to need less support over time, not more.
What happens if the individual’s needs change significantly?
We respond to changes in need promptly and transparently. If an individual’s needs increase, we notify the commissioner, request a review, and increase support in the interim where safe to do so. If needs decrease, we flag this at the next review. We never continue providing a level of support that is no longer appropriate โ either because it is insufficient or because it undermines the individual’s independence.
Do you support individuals through the transition from children’s services?
Yes. Transitions from children’s to adult services are a critical period and we have experience in supporting young people through this process. We engage early โ ideally 12 to 18 months before the transition date โ to develop the supported living plan, identify suitable housing, and build the support team so that the young person moves into adult life with a stable, familiar package already in place.
