We understand that arranging complex care can feel daunting. Our process is designed to be thorough, transparent, and centred on the individual โ so that every aspect of their care is properly planned before the first visit.

Referral and Initial Discussion
Contact us by phone on 07510 916481 or via our referral form. We discuss the individual’s diagnosis, current care arrangements, specific clinical needs, and what support is required โ and we liaise directly with NHS or social care teams where needed.

Comprehensive Home Assessment
Our experienced assessor visits the individual at home to carry out a thorough needs assessment โ covering clinical procedures, moving and handling requirements, equipment needs, daily routines, goals, and risk management.

Care Planning and Carer Training
We develop a detailed written care plan and clinical protocols. Where specific procedures are required, we ensure the assigned carers are fully trained and assessed in those tasks before care begins โ in partnership with the individual’s NHS team.

Care Begins With Ongoing Oversight
Care commences to the agreed plan. Our management team maintains close oversight through spot checks, regular care record reviews, and direct communication with the individual, their family, and their wider healthcare team.

Complex and Disability Care FAQs
Can you support someone with a tracheostomy or ventilator at home?
Yes. We provide complex care support for individuals with tracheostomies and those who are ventilator-dependent. All carers assigned to these individuals are specifically trained and written-assessed in tracheostomy care and ventilator management before being placed. We work closely with the individual’s NHS respiratory and community nursing team to ensure care is delivered safely and to protocol.
How do you ensure carers are trained for clinical procedures?
Every clinical procedure required in an individual’s care plan is identified during the assessment stage. We then ensure all assigned carers receive training from a qualified practitioner โ often in partnership with the NHS team โ and are written-assessed before the procedure is included in their care responsibilities. Clinical training is refreshed regularly and recorded in each carer’s personnel file.
Can you take on NHS Continuing Healthcare funded care packages?
Yes. We work with NHS Continuing Healthcare (CHC) funded packages, personal health budgets, local authority-funded packages, and privately funded care. We are experienced in working with ICB commissioning teams, social workers, and CHC coordinators to set up and manage care packages efficiently.
How do you manage moving and handling for individuals with complex needs?
Moving and handling is assessed in detail during the home assessment. We identify all equipment required โ hoists, slings, transfer boards, profiling beds โ and ensure the environment is set up correctly before care begins. All carers complete a moving and handling assessment specific to the individual and are supervised until competency is confirmed.
Do you provide 24-hour or live-in care for complex needs?
Yes. We provide waking night support, sleeping nights with responsive cover, and full 24-hour live-in care packages for individuals whose needs require continuous or overnight support. Live-in carers are specifically matched to the individual and receive the same clinical training as the day care team.
What happens if a carer is unwell and cannot attend a shift?
We maintain a pool of trained cover carers who are familiar with the individual’s care plan and clinical needs. In the event of a carer absence, we contact the cover carer and the individual’s family or representative as early as possible. For complex clinical packages, we never send an untrained carer as a substitute โ we escalate to management to ensure continuity of safe care.
